Please arrive 15 minutes early to fill out paperwork. You can print the paperwork and bring it with you by clicking on the links below. We ask that you wait to sign everything until you are in our office.

Please bring your insurance card to your appointment. Our services may not be covered under the terms of your insurance policy. Please contact your insurance company regarding covered services. Some plans require pre-authorization or the referral of a primary care physician. It is your responsibility to understand your plan requirements and to obtain any necessary authorization or referrals. 

Please note: All minors must have a parent or legal guardian with them for their first appointment.
